Need a medical card? It’s easy!! Let us do all the work!!

Documentation needed:

1. Oklahoma State Identification card or driver’s license.
2. Proof of Medicaid, Medicare, or Sooner Care if applicable.
3. If you are a 100% disabled veteran, we will need a letter from the VA stating that -- within the prior 6 months.
4. An active email address that you have access to, address where card will be mailed, and a working telephone number.

Procedure includes:

1. We will get all the paperwork completed that is necessary to start your medical recommendation process.
2. Once paperwork is received, we will set up your OMMA account. DO NOT CHANGE PASSWORD UNTIL YOU RECEIVE YOUR CARD!!
3. We will get a photograph of you, and this will be the picture that appears on your medical marijuana card.
4. We will submit your paperwork to the physician for your recommendation.
5. You will have a face-to-face phone conference with the physician.
6. Once we receive your recommendation, we will be in contact with you to submit your paperwork over to the state.
7. Once all fees are paid and your application has been submitted to OMMA, you can wait on your card to come in the mail.

If you are renewing your card, we suggest that you start that process 30 days prior to your current card’s expiration date. You will need the email address and password used to log into OMMA that was used for the card you currently have. If you do not have that information, you can contact OMMA via email support@mycomplia.com to get that information. If you do not have access to that email used previously, they can help you update that information. We cannot upload your new information to OMMA unless we have that username and password from your previous card. The prices and fees are the same as if you were applying for a medical card for the first time. All the previous documentation requirements and procedures apply to a renewal.
